Marine David O'Sullivan, a veteran of Afghanistan, died from a drug overdose.
Ian Smith, Senior Coroner for Stoke-on-Trent and North Staffordshire, concluded David died from a "drug-related death on a background of PTSD after military service and for which due to a number of failings he did not receive treatment".
